WHO WE ARE

Community Care Development Network (CCDN) comprises leaders in the community to help develop and maintain local programs. CCDN is a   faith-based non-profit 501c3 Organization specializing in healing and helping the whole family. Our purpose is to help heal the whole family through our community-based programs.

HUMAN SUPPORT SERVICES

​Human Support Services address immediate needs by providing access to food, essential supplies, and critical resources that support health and well-being. Through initiatives like Community Food Hubs and resource navigation, CCDN ensures families can meet basic needs while working toward long-term stability.

What community problems do we help solve?

Community Care Development Network (CCDN) operates through a comprehensive, community-driven model designed to address the root causes of instability while empowering families through four key service areas. Our Strengthening Families Model integrates Prevention, Access, Stability, and Violence Interruption to support individuals and families at every stage of their journey—meeting immediate needs while creating pathways to long-term success.

Our work is deeply rooted in understanding and addressing the social determinants of health (SDH)—the non-medical factors that influence health outcomes and overall quality of life. These include conditions such as income, education, employment, food access, housing, healthcare, and social inclusion. Research shows that these factors account for a significant portion of health outcomes and are key drivers of inequities in underserved communities.

At CCDN, we intentionally align our programs and services to address these complex challenges:

Human Support Services (Access)

We address food insecurity through Mobile Food Hubs and connect families to essential resources such as shelter, healthcare, financial assistance, and basic needs support. By removing barriers to access, we help stabilize households and improve health outcomes for families facing economic hardship.

Education Support Services (Stability)

We provide pathways to success through Inspire Education Academy, offering academic credentialing, workforce training, and career readiness opportunities. By addressing education and employment gaps, we empower individuals to achieve self-sufficiency and long-term economic stability.

Family Support Services (Prevention)

We proactively strengthen families through mentoring, parenting resources, life skills development, and counseling support. This approach supports early intervention, promotes healthy family dynamics, and helps prevent crises before they occur.

Community Outreach (Violence Interruption & Access)

We engage communities through events, pop-up food hubs, and strategic partnerships to ensure services are accessible and responsive. Our outreach efforts build trust, reduce barriers, and promote safe, connected, and resilient communities.

Through initiatives like Community Food Hubs, housing and resource navigation, workforce development, and partnerships with healthcare providers, CCDN addresses critical factors such as food insecurity, unemployment, housing instability, and access to care.

We recognize that achieving health equity requires coordinated action across all sectors. That is why CCDN remains committed to addressing both the immediate needs of families and the systemic barriers that perpetuate poverty and inequity.

Together, our holistic and data-driven approach fosters stability, builds resilience, and creates thriving, healthier communities for all.
